Two Licensed Services, One Standard of Care
Goshen Concept Care is licensed in Minnesota for 144G Assisted Living and 245D Home & Community-Based Services. We accept the CADI, Elderly, and Brain Injury waivers as well as private pay.
24-Hour Assisted Living (144G)
Round-the-clock residential care in our warm, family-oriented home in West St. Paul, including meals, medication management, and daily living support.
Homemaker Services (245D)
In-home support across Minnesota: light housekeeping, meal preparation, medication reminders, companionship, and help with daily routines.

We're a licensed Minnesota provider currently accepting referrals statewide, with quick answers on capacity and steady communication from first call to start of care.
