Choosing care for a parent, a spouse, or someone you support is one of the hardest decisions a family makes. It usually arrives at a stressful moment, like a hospital discharge or a sudden change in health. And after the third or fourth brochure, every agency starts to sound the same.
When families ask us where to start, we point them to three things. They are the same standards experienced case managers use, and any good agency in Minnesota should be glad you asked.
1. Licensing, screening, and training
Minnesota licenses the agencies that provide home care and community-based services. Two licenses come up most often:
- 245D is the Department of Human Services license for Home and Community-Based Services. It covers the waiver services that help people live at home and in the community, including homemaker services.
- 144G is the Department of Health license for assisted living.
Licensing matters because it sets enforceable standards for caregiver background studies, training, supervision, person-centered planning, and how concerns get reported and resolved. Ask any agency directly: What are you licensed for? How do you screen and train your caregivers? Who supervises care? A quality agency answers plainly, and is proud to.
2. Communication and responsiveness
The best caregiving in the world does not help much if nobody calls you back. Day to day, the difference between a great agency and a frustrating one usually comes down to communication. Do they respond quickly? Do they keep the family and the case manager informed when something changes? Is there a real person you can reach?
Ask how quickly they respond to a new referral or a question, how they share updates, and who your point of contact will be. An agency that is responsive in the first week tends to still be responsive in year two.
3. The right fit for the person
Care is personal. The right agency takes time to learn the individual: their routines, their preferences, their culture and language, and what a good day looks like for them. Then it builds the care around that person instead of fitting the person into a schedule. Consistency counts too. Familiar caregivers, wherever possible, beat a rotating cast of strangers.
A trustworthy agency also respects choice. The decision belongs to the individual and their family, and a good provider supports that decision rather than pressuring it.
